TREATMENT APPROACH
A multimodal approach is used to give optimum pain relief.
Routinely we start with simple and basic treatment and go up the ladder (as shown) as required. However, it is just a rough guideline. As each patient and each pain is different, a tailor-made treatment plan is formulated based on detailed consultation and evaluation of the patient's condition.
STEPS
-
Drugs
- Basic pain killers
- Stronger pain killers including opioid drugs like morphine for terminally ill patients
- Additional specific medication for chronic pain (pain transmission modulating medications)-
- Antidepressants (In small doses)
- Anticonvulsants
- Adjuvant Preparations
- Regenerative type Medications
-
Minor procedures performed on OPD Basis
- Tigger point injections
- Peripheral nerve blocks
- Intramuscular stimulation
- IV regional blocks
-
Basic International Therapies
- These are minimally Invasive procedures which give long term/ permanent pain relief.
- They fill in the gap between medical management and more Invasive operative procedures.
- They are done in the operation theatre under fluoroscopy ( specialized x-ray imaging) or sonography guidance as per international standards.
Commonly Performed Procedures
For Back Pain. Sciatica and Disc Problems
- Transforaminal epidural injection (nerve root block)
- Epidural adhesiolysis (in post-spine surgery)
- Facet joint injections/ median branch block (for localized back, buttock, groin, or thigh pain)
For joint Pain
- Intra-articular injections
- Joint nerve blocks
- Regenerative Therapy
CRPS. Vascular Insufficiency, Neuropathy, Cancer Pain
- Sympathetic blocks- stellate, coeliac, lumbar sympathetic, superior hypogastric
- Chemical/ thermal neurolysis (alcohel/RF ablation) - for permanent pain relief.
Advantages
- Targeted delivery of medications
- Minimally invasive procedures
- Day stay procedures- no admission required
- Very rare possibility of complications or long-term side effects
- Easy and safe to repeat.
- Good alternative for people who are not keen on surgery or are medically unfit to undergo surgery
